Ordered the 15 and 16mm as I and stretched to a 46fr Dittle. Screwed the 15 in first and, yes, you really should screw it in. That entry works best. Wore it off and on for a few days. Finally inserted the 16 and it screwed in just as easy. I was able to screw it down to the point it disappears into the meatus, then folded the ring to the side. A nice feeling wearing it inside my urethra for hours at a time. I found leaving it in overnight helped the stretch a bit more. Make a 17 or 18mm and I’m a buyer.
This is the best plug I've ever owned. It looks slightly different to the image shown, the screw thread is rounder and less deep than shown. Went for the 13mm, which is 2mm over the biggest sound I can comfortably manage. It's super tight tight but it screws in perfectly and stays in place with no pushing out or leaks. I can wear this 24/7 with no issues, just patience required to unscrew it.
